In my opinion, the best clicker training guide is Clickertraining: The 4 Secrets of Becoming a Supertrainer by Morten Egtvedt & Cecilie Koeste, top Clicker Training experts from Norway. Their book was first published in Norway but has now been released in ebook format along with five online bonus videos. Here's a video to give you an idea of what it covers.

Personally, I'm really impressed with their work and with clicker training as a dog training methodology. I like that it's based on scientific research and is very systematic.
It's effectiveness can be seen by how it's used to train more difficult subjects like marine mammals. Most of the training has to be done at a distance and so communication has to be so much more precise.
